Our Programs and Services

  • Senior Companion -The Senior Companion Program pairs individuals 60 and over with seniors and other people who have difficulty with daily living tasks. Through their assistance and friendship, Senior Companions help others live independently in their own homes rather than an institutional setting. Senior Companions volunteer for 20 hours per week and become part of a team of caregivers. In return, they receive a modest stipend plus meal and transportation reimbursement, and oftentimes, friendships that lasts a lifetime.   • TeleCare - TeleCare is a personal reassurance program for those who need to stay in touch. On a daily basis, ElderServe's TeleCare volunteers call elderly and homebound people. Volunteers call every morning (8:30-11:00 AM), Monday through Friday, to check on the well being of those in the program. If someone doesn't answer, follow-up calls are made, and if necessary, the TeleCare volunteer alerts neighbors, family, police and/or medical officials. To be a client, the requirements are: (1) Client must want to be called; (2) Client must be available/responsive to receive a phone call between 8:30-11:00 AM; (3) Clients need a "contact" person who keeps in touch on a regular basis; and (4) Clients contact our home office when they know they will not be available for their morning safety check.   • Client Services: Individual Services - This program is designed for individuals in targeted areas of Louisville, who may have special circumstances in their lives that need temporary social service. Assistance is provided finding the resource for needs ranging from post-hospital planning to housing, transportation, government benefits and family relationships.   • Client Services: Group Services - Programs are designed for people fifty and over and include creative arts, educational and self-help opportunities, and travel and trips. The Oak and Acorn Intergenerational Center located at 28th and Magazine Streets in West Louisville offers recreational services with additional intergenerational activities.   • CustomCare Home Service - CustomCare Home Service is not a maid service for able-bodied seniors, but a service for "assistance-in-living" care. Flexible, customized plans for individuals include: Personal Care (bathing, grooming, dressing), Home Management (light housekeeping, laundry, grocery shopping, light meal prep), Wake-up Services (bath, breakfast, medication reminder), Chore Services (window washing, carpet shampooing, general clean up). Costs for services are Chore Services (3 hour minimum) $60 per hour. For all other listed services: (3 hour minimum) $51; 4 hour in home visit - $64; 12 hour rate - $155; Bath Only - $28.   • Crime Victims Assistance - In existence since 1996 and funded through the Victim of Crime Act, the program provides assistance to seniors who have been the victims of violent crimes, or personal or financial abuse. Victims receive a comprehensive array of services from the beginning of the incident through the court process and beyond; all designed specifically for older people. Services include help with filing police reports, emergency repair, replacing identification, obtaining Emergency Protective Orders, working with police, crisis intervention, and counseling.   • Adult Day Care - ElderClub is located at the Oak and Acorn Intergenerational Center at 28th and Magazine Streets in West Louisville. The program is designed to provide adult day care services to high-need, homebound elderly who can utilize a medically supervised setting during the day to allow them to remain in the community and delay institutional or nursing home placement. The facility is licensed and accepts those who qualify for Medicaid and KIPDA subsidies. For more information, contact Harold Jones, Program Director at 776-3066.

  • SIT-UP (Supporting Independence Initiative) SIT-UP, located in Dosker Manor, is one of six nationally funded projects that enables the elderly and non-elderly residents of public housing to live independently and to prevent premature or unnecessary institutionalization.   • Nutrition Program - Lunches are provided Monday through Friday for over 250 seniors at ElderServe's Dosker Manor location, at the Oak and Acorn Center in West Louisville and for those who are homebound. For questions concerning assessment and participation in the on-site meals program, contact Ranney Tucker, 587-8673.
